Please complete the form below if your student's dismissal plan will change on any given day. Some examples: you will pick up your student early for a doctor's appointment, your student normally attends aftercare but will take the bus today, your student who normally takes the bus will be picked up by a parent/guardian at dismissal, etc. This helps ensure that your student goes to the proper location at dismissal!
